1. Participation Agreement
By joining SurgeVita, you agree to fully participate, complete assignments, and engage respectfully
with your coach and group.
2. Confidentiality
All sessions are confidential unless disclosure is required by law.
3. Liability Waiver
Coaching is a growth-focused service—not therapy or medical treatment. You are responsible for your
decisions and results. SurgeVita is not liable for direct or indirect outcomes.
4. Satisfaction Guarantee
Refunds are available only under the terms outlined in the Payment & Refund Policy.
5. Payment Terms
- Clients agree to pay in full (either up front or through a payment plan).
- We encourage you to reach out directly with concerns before initiating chargebacks. Chargebacks
outside the refund window may be contested and could result in collection or legal action.
- Payment plans must be completed in full, even if sessions are forfeited.
6. No Guarantees of Results
Your results depend on your commitment, consistency, and application. We provide tools and support,
but outcomes vary.
7. Media & Testimonial Consent
Sharing your story is optional. With written approval, SurgeVita may use testimonials or feedback for
promotional purposes.8. Communication Boundaries
- Support is provided during sessions and through designated communication channels.
- We typically reply within 24–48 business hours.
- SurgeVita does not provide emergency or medical services. If you are in crisis, please immediately
contact local emergency services (e.g., 911 in the U.S.) or a licensed mental health professional for
support.
9. Non-Transferable & Non-Deferrable
Enrollment cannot be transferred to another person or postponed to a later date. All sessions must
follow the timelines outlined here.
10. Intellectual Property
Feel free to share your personal takeaways and insights. However, all program materials (worksheets,
recordings, modules) remain proprietary to SurgeVita and cannot be copied, shared, or distributed
without permission.
11. Program Pauses
- Pauses may be granted in extraordinary cases (e.g., medical emergencies) at SurgeVita’s discretion.
- Typically limited to 30 days, but with documentation, up to 60 days may be considered.
12. Dispute Resolution
Disputes must first go to mediation in Wyoming. To initiate, contact **[email protected]**. If
unresolved, disputes will be settled by binding arbitration, not in court.
13. Mental Health & Participation
- SurgeVita does not provide therapy, counseling, or medical care.
- If you need support beyond our coaching scope (e.g., suicidal ideation, severe mental health
concerns), SurgeVita may end your enrollment and refer you to licensed professionals to ensure your
safety and access to appropriate care.
- A **pro-rated refund for unused sessions will be provided** if SurgeVita terminates enrollment for
mental health-related reasons, with a simple confirmation (e.g., a brief note from a licensed
professional, if available; no detailed medical records are required).
- Refunds are **not provided** for terminations due to disruptive behavior unrelated to mental health,
such as verbal abuse, harassment, or repeated interruptions that hinder group safety or program flow.- Participation is at SurgeVita’s discretion. If your behavior disrupts the program, you will receive written
notice via email and be invited to a private 1:1 discussion to address concerns. If issues persist,
enrollment may be terminated.
14. Catch-All Clause
- SurgeVita reserves the right to update policies, scheduling, or structure to protect program quality.
- Major changes (e.g., curriculum or session format) will always include at least 14 days’ notice and an
opportunity for client feedback.
- Minor updates (e.g., scheduling adjustments) will be communicated via email to keep you informed.
- We value your input on your coaching experience! Share feedback via our mid-program survey or by

Policy
1. Payment Options
- **Pay-in-Full**: Due at registration.
- **Payment Plan**: Automatic recurring payments billed every 30 days until the plan is complete.
2. Commitment
- When you enroll, you’re committing to the full program experience.
- Payment plans are **not cancellable** once the refund window has passed.
- All scheduled payments must be completed, even if sessions are missed or forfeited.
3. Refunds / Money-Back Guarantee
- We want you to feel confident in your investment. That’s why we offer a **21-day money-back
guarantee** that begins on the earlier of:
- (a) your first scheduled session, OR
- (b) 30 days after enrollment.
- To qualify, simply submit a written request within this period and show proof of **attendance** at a
scheduled session.
- After this window, **all payments are non-refundable**.
4. Program Start Deadline
- To keep your momentum strong, your first session must be scheduled within **30 days of
enrollment**.
- Programs can begin up to **14 days after enrollment**, as long as the first session is booked within
the 30-day window.
- In extraordinary cases (e.g., medical emergencies, bereavement), you may request a **one-time
14-day extension** with documentation.
- To help you succeed, we’ll send an **onboarding email within 7 days of enrollment** with scheduling
instructions and a quick summary of key policies. In the future, these will also be available at**surgevita.com/policies** for easy access.
- If no session is scheduled within the timeframe, your enrollment will be forfeited without refund.
5. Program Expiration
- Your program is designed to be completed within **16 weeks of enrollment**.
- If more than **four (4) sessions are missed**, remaining sessions are forfeited and the program will be
considered complete.
- In documented cases (e.g., medical emergencies, family crises), clients who email
**[email protected]** within 48 hours of a missed session may request up to a **two-week
extension**.
- Your commitment to payments helps us maintain program quality, even if sessions are missed.
6. Missed Payments
- If a payment fails, your program access will be paused until the account is current.
- You’ll receive a **one-time courtesy reminder** before any late fee is charged.
- A **$25 late fee** applies if payment is not received within 5 days of the due date after the reminder.
- If unpaid after **14 days**, sessions continue to expire on schedule and are considered forfeited.
Once your account is brought current, you can resume—but **no extensions will be given**.
7. Cancellations, No-Shows & Late Arrivals
- Sessions can be rescheduled with at least **1 hour’s notice**.
- The first missed/late/no-show session is waived as a courtesy.
- The **second missed/late/no-show** will result in a **$25 fee** before scheduling future sessions.
- The **third and subsequent missed/late/no-show sessions** will result in a **$35 fee** each.
- Fees may be waived for documented emergencies (e.g., medical issues) or valid reasons (e.g.,
unexpected work conflicts, technical issues with advance notice), at SurgeVita’s discretion.
- If you provide less than 1-hour notice or no communication, the session will be marked completed and
the fee applies.
8. Group Coaching Enrollment
- Groups are designed for **6–8 participants** to create powerful interaction and support.
- Groups will run as scheduled, even if fewer participants enroll. If a group is below 6, it may be
combined with another cohort—but the **curriculum and coaching hours will always remain the same**.
